The easiest way to use this instruction is to have two internet browser screens up, and switch between your view of this page and the Administration Page. - The Assessment Report is the means to tell Lacap what has happened in the assessment.
- The Report is about what working-contract conditions have been agreed with the patient.
- Please complete the Assessment Report on the day of the assessment.
- The format of the Report is indicative of the basic agreements that lacap expects you to arrive at with any patient - the working contract.
- The lowest form of agreement is merely that another meeting will take place - a further assessment.

The Application Process When the patient applies online, they create a record of that transaction - which is the basic information of the Assessment Report. In the Administration Section, once the patient in the front-end asks to go to the Appointment Manager, and once the management activate that record, the contact details and fee calculator details that they supply set up an Assessment Report in your view of the Administration Section. Before the initial consultation, you should view the Assessment Report and note the fee proposed. Separately, a basic record is created in the Appointment Manager, and the Appointment Manager will have let you have mail detailing the initial consultation details. In both records there are the patient contact details: name, mobile, email. If there is no patient record showing in your Assessment Report Section, phone or mail management for them to activate. When you take the assessment you will need to check with the patient that the contact details that they have supplied are correct, and you will need to agree a fee based on their application log. The other way that patients are listed up for assessment is via a direct telephone call with lacap, or via an email request. There is no practical difference for you, and the above comments will still apply. Before the Assessment It is a dispiriting fact that patients often do not turn up for initial consultations. It is not the medium that counts - they also do not turn up if things are arranged over the telephone, or even if they have organised things in person! You can do two things. A call or e-mail message personally acknowledging the booking can help establish a relationship before the meeting. You can also add to that contact that if the patient does not call/mail you 24 hours before the meeting, then you will assume that they are not coming along. Alternatively, be prepared to sit the hour out, anticipating that 1/3rd of initial consultations are no-shows. This is also why lacap will not charge you room fees for initial consultations booked in - so also the 48 cancel session rule does not apply. The Assessment Report Select the Assessment Report Record. Place you cursor above the number, & look to the right-hand side of the number row to check the patient name. 'Click' to select the appropriate record. Wait for the record to load. The Logic of the Assessment Report The purpose of the assessment report is to let lacap know how the assessment was concluded. If you scroll to the bottom of the Assessment Report, you will see five 'sign-off' options. Each of the 'sign-off' options dictates whether the fields above are required, optional or must not be completed. So do not worry too much - if you have a clear idea of what your sign off must be, then if you go wrong the system will prompt you for what other information is required, or what information defies the logical paths. One way to go at things while becoming confident is to sign off the report first, and then to keep on 'Sign off report' and follow the prompts until it is done. The sign-offs are - That you have taken the patient
- That the patient was a no show & you are not taking it further
- That you have met the patient but want him to be seen by another therapist
- That there has been a difficulty in finalizing an offer with the patient
- That the patient did not want to take your offer up.

If the patient is a no show, ignore everything and go straight down to that sign of option on the Assessment Report. In all other cases, you are asked to confirm that both you and lacap agree that this assessment report is your business & to do with your patient assessment. Please check of the boxes. - Patient Contact Details:
Obviously, patients can enter misleading, wrong or mistaken info. on their application. Please take the time to make sure the records are correct, and amend where required. - Was the assessment agreed?
Simple yes/no stuff driven by the logic of the 'sign-off'. - The offer that was made: Note: if you want to select more than one option in the drop-fields, hold control and select your multiple entries. The principal use of the mode & frequency fields is to set up the record correctly for any patient that you are passing to the 'Waiting Room'.
Unless further assessment is required, you will have to pass the matter to the management if you have been unable to agree a fee with the patient & if the assessment is otherwise complete. Please fill in the fields. - Did you accept the patient onto your list:
The 'Waiting Room' is the idea that if you have completed the assessment but have not agreed with the patient that you will be the consulting therapist, then enough information needs to be generated by the assessment report to enable another therapist & management to decide who might be suitable to take that patient on from there.
This is what the patient availability times are all about. As before, hold control while selecting multiple options, and use the notes for any brief information that might enable another therapist to decide to take on that referral. Also, indicate what venue the patient wants continuing meetings at. Sign offs & emails When the assessment reports are signed off standard mails are generated to the patient. The assessment is complete and I have accepted the patient onto my list. Dear Sample Patient Following your Initial Consultation, we are writing to confirm details of our offer to you. The therapist that you will be working with is Assessing Therapist. Please keep a note of the Contact Details: , A separate e-mail will be sent to you about with the details of your next meeting with Assessing Therapist. We have agreed a fee with you of £ 22.00 per session. Please arrange to make payment directly to your therapist after each session, whether by cheque or by cash. You should understand that if sessions that you have agreed to attend are missed by you or cancelled late, it is usual for the fee to be payable. Our offer for therapy is open-ended. You may work with your therapist for as long as the treatment takes to conclude. You are of course free to break-off at an earlier time. However, LACAP does reserve the right to undertake a three-monthly appraisal of our offer, and to withdraw the therapy if it is felt that matters are not progressing satisfactorily. We have agreed that you will take up therapy at a minimum of one session per week, subject of course to holidays and other natural absences. However, it is often helpful for the work if you meet more frequently with your therapist. This can be decided between you both. Similarly, please do negotiate the times of the sessions with your therapist. We trust that your questions will have been dealt with at the Initial Consultation. However, if you need further information, or have difficulties that cannot be raised directly with your therapist, then please contact the number given below. We also ask you to note that should you decide that you would rather make arrangements to meet with a different therapist at LACAP, we will make those arrangements after you have advised or discussed this option with Assessing Therapist.
